Why Patient-Partners?

Our Providers know more about bodies in general, but you know more about yours specifically. It's a partnership to figure out what makes sense for your health and the best plan to preserve long-term wellness.

What insurance do you accept?

The current healthcare system operates on insurance, which helps to alleviate costs for our patient-partners and allows us to work in compliance with our network of specialists. Therefore, we accept most commercial insurances and medicare, except state medicaid.

Why is there a membership fee?

We are dedicated to providing quality, compassionate and meaningful patient care. Our membership-based model helps us to sustain an adequate patient-doctor ratio to ensure that you receive the care you deserve, and the time it requires for your provider to truly listen, understand and talk with you (not at you).

What am I paying the membership for?

Individual membership plans start at $150 per month. We work with your insurance - just like a typical doctor’s office - but your membership allows us to provide the extended benefits of a patient-centered medical home. Our provider interacts directly with every patient, taking the time it requires to compassionately care for and properly treat them. Your initial visit is included with the membership fee.

Should you choose not to sign up for a membership, your insurance will be billed for the visit

What does the membership fee include?

A monthly membership fee covers the first visit and quality, comprehensive time invested in each patient’s health, including:

• In-house labs and blood draws
• Help successfully navigating the healthcare system - making sure follow-through happens on orders, imaging, referrals and medications
• Longer visits at the office with provider
• Shorter wait times - receive a call directly from the provider
• Remote medicine/telemedicine available to care for patients when they are traveling and need assistance
• Discounted access to proactive, sustainable wellness plans
• Same-day/week appointments

Who can join?

Anyone can become a member at Custom MD. Those committed to their health journey and willing to fulfill their part of the personalized care plan will get the most out of membership.
